Step 3: Deployment
The nurse begins duty at the patient's home, typically within 24 to 48 hours of the assessment. Urgent post-discharge and palliative cases are prioritised for same-day start when a nurse is available.
Step 4: Ongoing Monitoring
Daily health logs covering vitals, pain scores, food intake and medication compliance are maintained. A care coordinator follows up weekly and logs are shared with the oncologist at each visit.
